Wraparound Services.ĚýWraparound services that meet the needs of students and families are key to improving multi-generational outcomes. Each school, supported by the WLC, will have the flexibility to determine which wraparound services can best meet the needs of their students and school community, understanding that meeting the physical and behavioral health needs of the students and school community is vital to success.

Wraparound Services.ĚýThe Employer shall maintain minimum staffing ratios in compliance with the recommendations of the professional organizations as listed below.

Wraparound Services.ĚýHigh Fidelity Wraparound Services - The Wraparound Services model delivers services to children and families who are Medi-Cal beneficiaries with severe and multiple problems often being served by multiple agencies. Wraparound services includes treatment services, personal support services, and any other support necessary to maintain the child/youth in the family home or at the lowest level of appropriate care. Services are delivered/developed through an interagency collaborative approach that includes family participation and the family as an active team member.

Wraparound Services.ĚýWraparound Services must be prior authorized see Network Provider Manual, which is located on COUNTY's website, xxxxxxxxxxxx.xxx, under "Behavioral Health Services." Additional information can be found in DHCS Policy Letters and Informational Notices, which are available at the DHCS website. CONTRACTOR shall take all appropriate actions to ensure that no person employed by CONTRACTOR to provide services under this Agreement shall have committed, or been convicted of, any form of child abuse.

Wraparound Services.Ěý(1) Contractor shall administer fidelity Wraparound services to not less than 205 Members per month as described in the OHA System of Care Wraparound Initiative (SOCWI) guidance document found at: xxxx://0.xxx.xxx/1Rwu3Ym.Contractor shall ensure Fidelity Based Wraparound Care Coordination for Members 17 and younger for any of the following situations: (a) Placement in Secure Adolescent Inpatient Program (SAIP), Secure Children’s Inpatient Program (SCIP), (b) Psychiatric Residential Treatment Services (PRTS) or the Commercial Sexually Exploited Children’s residential program funded by OHA. (c) Children meeting local/regional System of Care Wraparound Initiative entry criteria. (d) Contractor shall ensure subcontractors (Day Treatment, PRTS, SAIP and SCIP providers) have an understanding of Wraparound values and principles and the provider’s role within the child and family team. Contractor shall ensure subcontractors collaborate and participate in the Wraparound process. (2) Contractor shall ensure that the ratio of care coordinators to families served is no greater than 1:15. (3) Contractor shall ensure that core values and principles described in ORS 418.977 are followed in delivering Wraparound services and supports, Wraparound Care Coordination and Team Facilitation. (4) Contractor shall ensure that SOC Wraparound services and supports include Family Partners and Young Adult Peer Supports, as appropriate. Such Family Partners and Young Adult Peer Supports must have experience navigating the mental/behavioral health, child welfare, or juvenile justice system with a child, and be active participants in the Wraparound process. Family peer support partners engage and collaborate with systems alongside the family/parent. Contractor shall refer to the System of Care guidance document on expectations for family support partner and Young Adult Peer Support expectations. (5) Contractor shall maintain a Wraparound Review Committee which shall review all referrals submitted to the Wraparound program and determine eligibility for entry into the program. (6) Contractor shall provide non-health related services, supports and activities that can be expected to improve a member’s mental health condition including flexible funding as defined by the SOCWI guidance document. Wraparound Services.ĚýEOC Business Services Account Specialist (BSAS) will conduct job search activities 15 with all students and assist with the development of individual resumes. Initially the BSAS will 16 meet with each student and discuss which construction path they want to pursue. At this point 17 the requirements are reviewed and an initial plan is put in place, allowing the student to continue 18 to conduct research to determine if they made an appropriate selection. The BSAS will then 19 facilitate job search activities and referrals along with additional assignments being given. 20

Wraparound Services.ĚýProvider shall administer wraparound care coordination services to Fidelity, consistent with the obligations set forth in Exhibit M of the CCO Contract. In particular, Provider shall: 1.1.1 Ensure certified providers administer the Child and Adolescent Needs and Strengths Assessment (“CANSâ€�) Oregon to members, consistent with the reporting requirements set forth in Exhibit M of the CCO Contract; 1.1.2 Ensure its providers and staff are trained in integration and foundations of Trauma Informed Care, recovery principles, motivational interviewing, assessing for Adverse Childhood Experiences, and rendering services in a Culturally and Linguistically Appropriate manner; 1.1.3 Adhere to applicable elements of the System of Care Wraparound Initiative Guidance Document published by the OHA; and 1.1.4 Complete required documents for each enrolled youth and their family pursuant to the Fidelity model.

Wraparound Services.ĚýThe Employer shall maintain minimum staffing ratios in compliance with the recommendations of the professional organizations as listed below. 28.1.1. Nurses: EPIC shall provide nursing services for twenty (20) hours per week.

Wraparound Services.ĚýThis funding only may be used to support Isolation/Quarantine for families including rent, mortgage, utilities, groceries, etc. For COVID-19 funding from ELC CARES for Infection Prevention, Case Investigations, Contact Tracing, and violation monitoring. The inability to meet the following metrics will elicit the following response from MDHHS related to this funding: â€� Technical assistance â€� Corrective action/performance improvement plans with MDHHS â€� Required support from MDHHS Any single procurement of over $4,000 should be vetted with MDHHS prior to purchase. â€� The LHD will designate a staff member or members responsible for leading infection control assessments. â€� The LHD will share the contact information of their infection control lead(s) with the MDHHS Infection Prevention and Resource and Assessment Team (iPRAT) for coordination purposes at [email protected]. â€� LHDs shall submit ICARs to MDHHS at This data will be reported quarterly and sent to program office via email. Allowable expenses include staffing, IT, communications, computers and or phones or other office needs. Funding cannot be used for clinical care or research. PROJECT: ELC Regional Lab ELC Regional Lab funds are dedicated to developing lab, epi, and other public health efforts to modernize and expand testing and response capabilities of pandemics with special emphasis on the responses to COVID-19. The Regional laboratory system is intended to serve as a “hub-and-spokeâ€� model in conjunction with the state public health laboratory to rapidly respond to community needs. Funding is expected in personnel, equipment, overhead (discreationary of county or district needs if in support of public health), and lab supplies/consumables or materials that directly support sampling for return to the lab.
