Service Reports Clause Examples
The Service Reports clause requires the service provider to regularly document and deliver reports detailing the work performed, progress made, or issues encountered during the provision of services. Service Reports.ÌýPaymentus shall provide Client with reports summarizing use of the Services by Users for a given reporting period.

Service Reports.ÌýThe Contractor shall be required to provide a signed service report after every service performed under this contract as certification of services performed. Service reports provided shall be inclusive of but not limited to, trouble shooting, repairs, and preventative maintenance. After each site visit, the Contractor shall document in writing the findings of each inspection/preventative maintenance service and any action(s) performed for each item/equipment serviced under this contract. The report shall be submitted to the District’s Maintenance Representatives upon completion of work and prior to leaving the jobsite. For emergency site visit, the report shall be furnished within twenty-four (24) hours of service completion. Service Reports.ÌýGRANTEE shall submit service delivery reports, programmatic performance reports and other reports to the extent requested by OAG, in a format and on a timely basis, as established by the OAG. XXXXXXX will submit other reports as requested by the OAG.

Service Reports.ÌýThe Contractor shall furnish the Authorized User with service reports for all Maintenance and warranty work upon completion of the services. The service reports shall include the following information in either electronic form as designated by the Authorized User:
1. Date and time Contractor was notified;
2. Date and time of Contractor’s arrival;
3. Make and model of Production Printer;
4. Description of malfunction reported by Authorized User;
5. Diagnosis of failure and/or work performed by Contractor;
6. Date and time failure was corrected by Contractor;
7. Type of service � Maintenance or warranty
8. Charges, if any, for the service; and Contractor shall provide OGS with an annual report on the 15th day of January of each year of the Contract detailing all Maintenance and warranty work conducted under the Contract for both State Agencies and Non-State Agencies during the immediate prior calendar year. The report shall include the name of the State or Non-State Agency and all of the information above (1-8) and be submitted electronically in Microsoft Excel 2010 or lower version unprotected, via e-mail to the attention of the OGS NYSPro contract administrator. The report shall list Authorized Users in alphabetical order under the following categories: 1. State Agencies; and, 2. Non-State Agencies.�
Service Reports.ÌýThe Quarterly Service Report shall be detailed by each Eligible User and a summary report of the details for the contract as a whole. The format of the report and level of detail shall be mutually agreed upon by the Contractor and the Department Contract Manager and shall include, at a minimum, the following:
a) Contractor’s name
b) Reporting period
c) Eligible User’s Name
d) Description of services provided for each Eligible User per quarter
e) Total dollar value of services provided per quarter
Service Reports.ÌýIt is the Supplier’s responsibility to maintain a record of all Service Reports during the term of this Agreement. On each Service Report, indicate the following: â€� The facility name, address, number, and date of service. â€� The type of service call: preventative maintenance, call-back/follow-up, or emergency service. Note: All Service Reports must be signed off by the Local Government representative. â€� Who initiated the service call. â€� The hours worked at the facility. â€� The work carried out during the visit. â€� Any defects, abnormalities in the system, required repairs, and any replacement parts with the name, part number, and the price. â€� Any parts installed along with the room location, cost, and the items listed in the specifications.
Service Reports.ÌýFirst Billing shall provide Client with reports summarizing use of the Services by Users for each reporting period.
Service Reports.ÌýThe Service allows the User to download various Service Report(s). Each report may only be downloaded once. Service Reports include:
a) A Jpeg image of the Service Dashboard;
b) A TWBX file for the Dashboard which can be viewed in a Tableau Reader;
c) A CSV file of the data contained within the Service Dashboard for use within a range of GIS and BI applications; Unless otherwise agreed in writing with the Service Provider, all Service Reports shall be deleted on expiry of the Customer Licence Subscription. Please refer to clause 21.5 regarding the Service Reports and the Customers obligations on Termination or expiry of the Term.
