Personnel Action Form.ÌýA form issued upon employment which does not constitute a contract but is advisory to the employee concerning date of employment, pay range and step, special terms of employment where applicable, job site, job classification, date of hire, and date of last physical exam. Probationary Period: Upon hire an employee’s probationary period shall continue for ninety (90) working days. During the probationary period, an employee may be dismissed for lack of job skills or other just cause. Resignation: A voluntary action by an employee to sever employment with the School District. Superintendent: Superintendent of Schools or designee. Termination: Discharge or any other action involuntarily severing employment with the School District. Examples of Personnel Action Form in a sentence

  • For employees who are retiring, resigning, being discharged, or laid off, the District shall show the last day worked as the termination date on the Personnel Action Form which is also the last day physically on the job.

  • When changes, additions, or deletions are made on a wage position, all pertinent budget information must be included on the Personnel Action Form.

  • Training scheduled more than five (5) days in advance, regularly scheduled shift changes, transfers as documented on a Personnel Action Form 14 and promotions shall not apply to this consecutive off day provision.

  • Work Progress Schedule means the continually updated time schedule prepared and monitored by Contractor that accurately indicates all necessary appropriate revisions as required by the conditions of the Work and the Project while maintaining a concise comparison to the Baseline Schedule.
