Eligible Patients definition

Eligible PatientsÌýmeans the estimated number of potential Care Plus patients for which a PHO received Care Plus funding according to the Care Plus funding formula.
Eligible PatientsÌýmeans those patients designated as eligible recipients in the gift instrument or other documentation establishing the Fund or, to the extent such designation is not made or is no longer applicable for any reason, patients who demonstrate financial need as determined by the Hospital following examination of available income, assets and such other information as may be required.

All Covered Entity patients who are Medicaid beneficiaries and for whom claims for pharmaceuticals are reimbursable by a state fee-for-service Medicaid program are expressly excluded from this definition.

  • Those persons who are determined not to be Eligible Patients shall be processed in accordance with Hospitalsâ€� billing and collection policies.

  • For the Contract Period, the County has established a budget allocation (the “Budget Allocationâ€�) for each such Non-County Trauma Hospital providing medical care to Eligible Patients (as defined below) during the Contract Period.

  • Eligible Patients who have received a Structured CDM Review between 01 January 2020 and 31 May 2020 will be eligible for a second review in 2020 once there is at minimum at least 4 months between reviews.

  • For those Eligible Patients whose prescriptions are reimbursable by a Private Insurer, Walgreens is responsible to process and bill such Private Insurer at the existing Contracted Rates.

  • Pharmacy will provide the designated pharmaceutical drug product or device for which the respective Customer will provide savings (“Covered Drugâ€�) to Eligible Patients in the same manner as such services are provided in the ordinary course of business absent this Agreement, without preference or discrimination.

  • Contractor shall provide Trauma Services, as defined below to Eligible Patients.

Eligible PatientsÌýmeans those patients who are eligible to participate in any hypofractionated proton therapy protocol and who would otherwise receive IMRT treatment;
