Dependent on or addicted to drugs or alcohol. It is an illness in which a person is dependent on alcohol and/or a controlled substance regulated by state or federal law. It can be a physiological (physical) dependency or a psychological (mental) dependency or both. People with Chemical Dependency usually use drugs or alcohol in a frequent or intense pattern that leads to: � Losing control over the amount and circumstances of use � Developing a tolerance of the substance, or having withdrawal symptoms if they reduce or stop the use � Making their health worse or putting it in serious danger � Not being able to function well socially or on the job Chemical Dependency includes drug psychoses and drug dependence syndromes. State and federal law require that the copays and coinsurance for medically necessary outpatient and inpatient services provide to treat chemical dependency will be no more than the copays and coinsurance for medical and surgical services. Prescription drugs to treat chemical dependency are covered under the same terms and conditions as other prescription drugs covered under this plan. A request for payment from us according to the terms of this plan. The amount you pay for covered services after you meet your deductible. Coinsurance is always a percentage of the allowed amount. Coinsurance amounts are listed in the Summary of Your Costs.
